Private Pay Patients
In the event you do not have insurance coverage and must pay for your services, we require payment at the time services are rendered. We accept cash, checks, Mastercard and Visa and offer a self pay discount. If you are unable to pay in full, please contact our office prior to your visit to make payment arrangements.
Insurance Filing Policies
In order to serve you well and to avoid misunderstandings, we would like to inform you of our office policies regarding the filing of insurance claims. We have made prior arrangements with many insurers and health plans to accept an assignment of benefits.
This means that we will bill those health plans for which we have an agreement and will only require you to pay your co-payment at the time of service. While we are pleased to be able to provide service to you, it is very difficult to keep track of all the individual requirements. We recommend that you contact your insurance company or human resources at your place of employment with specific questions regarding your plan benefits.
If at any time there is a change in your insurance plan please notify the front office staff so that your insurance claims are filed on your behalf in a timely manner.
Please be aware that different health insurance plans have different requirements regarding deductibles and co-pays. It is the patient’s responsibility to check with your insurance plan and verify your coverage and what your financial responsibility will be at each visit. Your deductible and/or co-pay will be collected prior to your being seen for your appointment. We do not have payment plans, but we do accept most major credit cards.
At this time, we are not accepting Medicare.
